Purpose of the Study:

  • To evaluate the correlation between intraoperative packed red blood cell (PRBC) transfusion and adverse outcomes in Spanish cardiac surgery patients.

Main Methods:

  • Retrospective, observational, multicenter study.
  • Analysis of 927 cardiac surgery patients from 24 Spanish hospitals (2007).
  • Comparison of transfused versus non-transfused patients using multivariate analyses.

Main Results:

  • PRBC transfusion associated with increased risk of acute kidney damage (72h post-op).
  • Transfused patients experienced prolonged mechanical ventilation and required hemodynamic support.
  • In-hospital mortality rates increased (Adjusted OR: 1.30; 95% CI: 1.19-1.42) with longer hospital stays (approx. 4 days).

Conclusions:

  • Intraoperative PRBC transfusion independently predicts higher risk of early acute kidney damage.
  • Transfusion is linked to prolonged mechanical ventilation, need for hemodynamic support, and reduced short-term survival.
  • Intraoperative transfusion is associated with significantly longer hospital stays.
