Background:

Rising rates of obesity and bariatric surgery necessitate exploring psychosocial factors.
Past abuse is an increasingly recognized contributor to obesity.

Purpose of the Study:

To investigate the impact of past abuse on a patient's post-bariatric surgery recovery.
To highlight the role of psychiatric evaluation in managing psychosomatic symptoms after gastric bypass.

Main Methods:

Case study of a 37-year-old female post-laparoscopic Roux-en-Y gastric bypass with intractable nausea and vomiting.
Psychiatric consultation for psychosomatic etiology evaluation after ruling out medical causes.
Utilized brief psychodynamic psychotherapy and pharmacotherapy.

Main Results:

Patient revealed a history of teen date rape leading to significant weight gain and obesity.
Symptoms resolved following psychiatric intervention and medical workup.
Patient remained asymptomatic at a 2-month follow-up.

Conclusions:

Screening for abuse history with tools like the Weight and Lifestyle Inventory (WALI) is recommended pre-surgery.
Individuals with abuse history require thorough screening and integrated psychiatric follow-up care.
Psychiatric involvement is a valuable adjunctive treatment during the medical workup phase.
