Background:

  • A 70-year-old female presented with chronic cough and intermittent fever.
  • History of thoracic surgery for tuberculosis 35 years prior.
  • Chest CT revealed a 4.5 cm soft tissue mass in the right lower lobe, suspected to be a foreign body.

Observation:

  • Conventional bronchoscopy failed to identify the foreign body due to bronchial stenoses.
  • Multiple bronchial stenoses were present secondary to chronic inflammation.
  • 3-dimensional CT with virtual bronchoscopy was employed for precise localization.

Findings:

  • Successful identification and removal of a pulmonary foreign body (gauze).
  • Utilized 3-dimensional CT and virtual bronchoscopy for navigational guidance.
  • Patient experienced rapid resolution of symptoms post-removal.

Implications:

  • 3-dimensional CT with virtual bronchoscopy is a valuable tool for locating and removing difficult pulmonary foreign bodies.
  • This technique can overcome limitations of conventional bronchoscopy in cases of bronchial stenosis.
  • Offers a minimally invasive solution for complex intrabronchial foreign body retrieval.
