Background:

  • Functional gastrointestinal disorders, including irritable bowel syndrome, show known brain microstructural changes.
  • The white matter (WM) microstructure in postprandial distress syndrome (PDS) remains understudied.

Purpose of the Study:

  • To investigate white matter (WM) microstructural alterations in patients diagnosed with postprandial distress syndrome (PDS).

Main Methods:

  • Diffusion tensor imaging (DTI) was utilized on 36 PDS patients and 36 healthy controls.
  • Tract-based spatial statistics analyzed fractional anisotropy (FA), mean diffusivity (MD), axial diffusivity, and radial diffusivity (RD).
  • Non-parametric permutation tests and threshold-free cluster enhancement addressed correlations and multiple comparisons.

Main Results:

  • PDS patients exhibited increased FA and reduced MD/RD in several WM tracts, including the corona radiata and corpus callosum.
  • Accounting for anxiety and depression largely diminished these differences, except in the corona radiata.
  • No significant correlations were found between DTI measures and symptom severity or disease duration.

Conclusions:

  • Preliminary evidence suggests white matter (WM) microstructural changes in PDS patients.
  • A portion of these observed brain changes may be attributable to elevated psychosocial distress in the PDS cohort.
