Background:

Tonsillectomy is a common procedure with significant postoperative pain.
Postoperative bacterial infection is a suspected cause of pain and morbidity.
Previous studies suggested antibiotics might reduce negative outcomes.

Purpose of the Study:

To evaluate the effectiveness of perioperative antibiotics in reducing pain and other morbid outcomes following tonsillectomy.
To synthesize evidence from randomized controlled trials on antibiotic use in tonsillectomy.

Main Methods:

Systematic review and meta-analysis of randomized controlled trials.
Searched multiple databases including Cochrane, PubMed, EMBASE, CINAHL, Web of Science.
Included 10 trials with 1035 participants, assessing pain, analgesia, hemorrhage, fever, and recovery.

Main Results:

Antibiotics did not significantly reduce pain or the need for analgesia.
No reduction in significant or total secondary hemorrhage rates was observed.
A significant reduction in fever was noted with antibiotic use (RR 0.63, P = 0.002).

Conclusions:

Current evidence does not support the routine use of antibiotics to reduce pain, analgesia requirements, or hemorrhage after tonsillectomy.
Methodological limitations in trials may bias results.
