Summary

A new assay using short tandem repeat (STR) loci efficiently screens for human leukocyte antigen (HLA)-matched siblings for hematopoietic stem cell transplantation (HSCT). This cost-effective method accurately identifies HLA-identical siblings, improving donor selection.

Area of Science:

  • Immunogenetics
  • Molecular Biology
  • Transplantation Science

Background:

  • Identifying human leukocyte antigen (HLA)-matched sibling donors for hematopoietic stem cell transplantation (HSCT) is crucial but often hindered by time, cost, and insurance limitations.
  • Current HLA matching methods can be resource-intensive, impacting the accessibility of potentially life-saving transplants.

Purpose of the Study:

  • To develop and validate a novel assay for efficiently determining HLA identity in sibling pairs using short tandem repeat (STR) loci.
  • To assess the accuracy, sensitivity, and specificity of the STR-based assay for identifying HLA-matched siblings for HSCT.

Main Methods:

  • Development of a panel of nine informative STR loci within the HLA complex.
  • Validation of the STR panel in 88 family workups, involving 132 related donor and recipient typing comparisons.
  • Assessment of STR allele identity against established HLA typing for accuracy.

Main Results:

  • The STR assay demonstrated 100% specificity and 91% sensitivity in identifying HLA-matched siblings.
  • All sibling pairs with identical STR alleles were confirmed as HLA identical.
  • Mismatches in STR alleles accurately predicted HLA non-identity, with 48/48 mismatched pairs identified.
  • The assay also provided valuable data for resolving ambiguous HLA haplotype inheritance.

Conclusions:

  • The developed HLA STR assay is a reliable, rapid, and cost-effective tool for screening potential sibling donors for HLA identity.
  • This assay can significantly improve the efficiency and reduce the cost associated with donor selection for HSCT.
  • The method offers a practical solution to overcome limitations imposed by traditional HLA typing methods and insurance reimbursement caps.
