Reliably profiling primary care physicians (PCPs) is challenging with single measures. Combining measures into composites improves reliability for some conditions, but not all, and profiling practice groups offers little benefit.
Area of Science:
- Health Services Research
- Quality Improvement
- Medical Informatics
Background:
- Performance measures are crucial for assessing primary care physicians (PCPs).
- Reliability of individual PCP profiles is often compromised by small patient panel sizes.
- Alternative methods like composite measures and group profiling are explored to enhance reliability.
Purpose of the Study:
- To evaluate the reliability of individual PCP performance profiles.
- To determine if composite measures or practice group profiling can improve reliability.
- To assess the impact of sample size on performance measure reliability.
Main Methods:
- Cross-sectional analysis of electronic health record data from April 2009 to May 2010.
- Inclusion of patients with diabetes, congestive heart failure, ischemic vascular disease, and preventive care needs.
- Evaluation of 14 diabetes, 9 CHF, 7 IVD, and 4 preventive care measures across 163 PCPs in 17 clinics.
Main Results:
- Single performance measures rarely achieved 80% reliability for individual PCPs (0%-60%).
- Single-disease composites improved reliability for diabetes and preventive care (74.5%, 76.7%), but not CHF or IVD.
- An overall composite measure allowed reliable profiling of 85.3% of PCPs; practice group profiling showed limited improvement.
Conclusions:
- Individual performance measures are generally unreliable for profiling PCPs.
- Composite measures can enhance reliability for specific conditions, but not universally.
- Profiling practice groups does not substantially improve reliability compared to individual PCP assessment.

Introduction to Documentation and Reporting
Nursing documentation records essential information and details regarding a patient's care and treatment in written or electronic form. It is a critical aspect of nursing practice that involves documenting assessments, interventions, outcomes, and other relevant details about a patient's health status.
