Background:

  • Lateral orbital wall fractures can lead to severe visual impairment, including blindness.
  • Management of post-traumatic visual loss, particularly optic nerve compression, remains challenging.
  • Traditional treatments like lateral canthotomy and corticosteroids may not always be effective.

Observation:

  • Two cases of visual loss following lateral orbital wall fracture are presented.
  • Case 1: Retrobulbar hematoma with optic nerve compression, unresponsive to medical management.
  • Case 2: Immediate blindness due to bony optic canal fracture.

Findings:

  • Extradural decompression of the orbit and optic nerve was performed using a lateral facial approach in both patients.
