Summary

Parathyroid carcinoma, a rare cause of hyperparathyroidism, often presents with high calcium and parathormone levels. Surgical excision is the standard treatment, but local recurrence is common.

Background:

Parathyroid carcinoma is an infrequent cause of primary hyperparathyroidism, accounting for approximately 1% of cases.
This review synthesizes current literature and presents three case reports on parathyroid cancer.

Observation:

Diagnosis is typically made post-operatively, as symptoms mimic benign adenomas.
Elevated serum calcium and parathormone levels are characteristic of parathyroid carcinoma.
Imaging modalities lack definitive diagnostic capability for malignancy.

Findings:

Fine needle aspiration is contraindicated due to the risk of tumor seeding.
Radical surgical excision, including ipsilateral thyroid lobectomy, is the primary treatment.
High rates of local recurrence are observed following surgical intervention.

Implications:

Parathyroid carcinoma necessitates specialized surgical management in expert centers.
Early and accurate diagnosis remains a clinical challenge.
Further research into diagnostic and therapeutic strategies is warranted.
