Background:

  • 18F-FDG-PET imaging shows potential for monitoring treatment response in pulmonary sarcoidosis.

Observation:

  • A case report details a patient with severe earlobe sarcoidosis.
  • The patient was treated with infliximab.
  • 18F-FDG-PET was utilized to visualize the extent of the disease.

Findings:

  • 18F-FDG-PET successfully visualized extrapulmonary sarcoidosis, even in small lesions.
  • A reduction in 18F-FDG uptake on PET scans correlated with clinical improvement.
  • This suggests 18F-FDG-PET is a viable tool for assessing treatment efficacy.

Implications:

  • This case highlights the utility of 18F-FDG-PET in monitoring infliximab therapy for sarcoidosis.
  • The findings support the use of imaging to guide treatment decisions in refractory sarcoidosis.
  • Early detection and monitoring of extrapulmonary sarcoidosis may be improved with this approach.
