Summary

A severe Capnocytophaga canimorsus infection, often from dog bites, caused shock and organ failure in a patient. Prompt antibiotic treatment and supportive care led to recovery without amputation.

Characterization of three strains of Capnocytophaga canis isolated from patients with sepsis.

Background:

  • Capnocytophaga canimorsus is an opportunistic Gram-negative bacterium, typically associated with dog or cat bites.
  • Severe infections can lead to sepsis, disseminated intravascular coagulation (DIC), and multi-organ failure, particularly in immunocompromised individuals.

Observation:

  • A 49-year-old male presented with fever, leg pain, purpura, cyanosis, shock, DIC, and liver/kidney failure.
  • A cat scratch wound and peripheral blood smear showing Gram-negative rods phagocytosed by neutrophils suggested Capnocytophaga canimorsus.

Findings:

  • The patient received ampicillin/sulbactam and catecholamines, requiring hemodialysis for 3 weeks.
  • Necrotic toes showed improvement, avoiding amputation. The patient was discharged with mild leg sensory impairment.

Implications:

  • This case highlights the potential severity of Capnocytophaga canimorsus infections, even in non-immunocompromised hosts.
  • Early recognition, diagnosis, and aggressive management are crucial for improving outcomes in severe Capnocytophaga canimorsus sepsis.
