Summary

Anterior segment swept-source optical coherence tomography (OCT) offers repeatable tear meniscus measurements. This quantitative method for tear fluid evaluation correlates with Schirmer test results but not tear film breakup time.

Area of Science:

  • Ophthalmology
  • Biomedical Optics
  • Tear Film Dynamics

Background:

  • Accurate assessment of tear fluid is crucial for diagnosing and managing dry eye disease.
  • Traditional methods for tear film assessment have limitations in precision and repeatability.
  • Optical coherence tomography (OCT) offers a non-invasive approach to visualize ocular structures.

Purpose of the Study:

  • To evaluate the repeatability of tear meniscus measurements using anterior segment swept-source OCT.
  • To explore the correlation between tear meniscus dimensions and established tear film assessment tests.
  • To determine the relationship between OCT-derived tear meniscus parameters and tear film breakup time (TBUT) and Schirmer test (ST) results.

Main Methods:

  • Prospective, observational, cross-sectional study involving 26 healthy subjects.
  • Anterior segment swept-source OCT was used for sequential vertical raster scans to measure tear meniscus height, area, and volume.
  • Measurements were repeated by the same grader, and images were analyzed by two masked graders to assess intergrader reliability.

Main Results:

  • High intergrader reliability (intraclass correlation > 0.95) was observed for all OCT tear meniscus parameters.
  • Significant correlation was found between OCT tear meniscus measurements and Schirmer test scores (P < .05).
  • No significant correlation was found between tear film breakup time and any measured tear meniscus parameters (P > .05).

Conclusions:

  • Anterior segment swept-source OCT provides a repeatable and non-invasive method for quantitative tear fluid assessment.
  • OCT measurements of the tear meniscus show good reliability and correlate with Schirmer test results.
  • Swept-source OCT is a practical tool for evaluating tear fluid dynamics in clinical settings.
