Summary

Patellar resurfacing in total knee replacement offers equivalent pain and satisfaction to nonresurfacing. However, it significantly reduces the need for subsequent surgeries related to patellofemoral complications.

Main Results:

  • Sixteen RCTs involving 3465 knees were analyzed; 1710 underwent patellar resurfacing and 1755 did not.
  • The knee component of the KSS was significantly higher in the resurfacing group (p=0.005), but function component and other scores showed no significant difference.
  • While anterior knee pain and patient satisfaction were similar between groups, patellar resurfacing significantly reduced patellofemoral complications (p=0.02) and reoperations for anterior knee pain (p<0.00001).

Conclusions:

  • Patellar resurfacing in TKR yields outcomes comparable to nonresurfacing regarding anterior knee pain and patient satisfaction.
  • Crucially, patellar resurfacing significantly decreases the incidence of subsequent surgical procedures related to patellofemoral complications.
  • Further research with long-term follow-up and patella-specific outcome measures is recommended for modern prostheses.
