Summary

Topical ophthalmic ointments containing mineral oil can cause lipoid pneumonitis. Discontinuation of Lacri-Lube resolved aspiration pneumonitis in a patient with nocturnal coughing and lung infiltrates.

  • Symptoms began after using Lacri-Lube, an ophthalmic ointment.
  • Bronchial washings revealed lipid-laden macrophages.

Findings:

  • Lacri-Lube aspiration led to lipoid pneumonitis.
  • The ointment's mineral oil and petrolatum content were implicated.
  • Cough and lung abnormalities resolved upon discontinuing the ointment.

Implications:

  • This is the first reported case of pulmonary complications from Lacri-Lube.
  • Ophthalmic medications, topical or systemic, can have diverse pulmonary effects.
  • Clinicians should consider lipoid pneumonitis in patients using ophthalmic ointments with respiratory symptoms.
