Summary

Unintentional stimulation of the ethmoidal nerve during computer-assisted sinus surgery can trigger a dangerous bradycardia and asystole. Surgeons must be aware of this reflex to prevent adverse cardiac events.

Observation:

  • A 53-year-old female patient experienced significant heart rate and blood pressure drops during CAS ethmoidal sinus surgery.
  • Transient asystole occurred during the procedure, resolving upon instrument removal.
  • Bradycardia recurred with instrument reinsertion, necessitating atropine for completion.

Findings:

  • This case highlights the potential for direct ethmoidal nerve stimulation via CAS probes to cause exaggerated diving reflex.
  • The incidence of this reflex during neurosurgical procedures is reported between 10-18%.
  • This is the first reported instance of direct ethmoidal nerve stimulation during paranasal surgery causing asystole.

Implications:

  • Exaggeration of the diving reflex, even if typically cardioprotective, can be detrimental.
  • Awareness and monitoring are crucial to manage bradycardia and asystole during CAS sinus surgery.
  • This reflex may be implicated in unexplained cardiorespiratory events, including sudden death.
