Summary

Delayed Sumithion (fenitrothion) intoxication occurred in a patient due to retarded metabolism. This case highlights the importance of considering delayed toxic effects in organophosphate poisoning.

Background:

Organophosphate pesticides like Sumithion (fenitrothion) are commonly used but can cause poisoning.
Delayed onset of symptoms in pesticide ingestion cases can complicate diagnosis and treatment.

Observation:

A 52-year-old male ingested Sumithion and alcohol, presenting with somnolence but initially lacking typical organophosphate poisoning symptoms.
Fasciculations and salivation, characteristic early signs, appeared 40 hours post-ingestion.
Blood concentrations of Sumithion were monitored over time.

Findings:

Pharmacokinetic analysis using a 2-compartment model suggested retarded metabolism of Sumithion.
The delayed appearance of symptoms correlated with the observed slow metabolic clearance of the pesticide.
