Summary

Delayed Sumithion (fenitrothion) intoxication occurred in a patient due to retarded metabolism. This case highlights the importance of considering delayed toxic effects in organophosphate poisoning.

Area of Science:

  • Toxicology
  • Pharmacokinetics

Background:

  • Organophosphate pesticides like Sumithion (fenitrothion) are commonly used but can cause poisoning.
  • Delayed onset of symptoms in pesticide ingestion cases can complicate diagnosis and treatment.

Observation:

  • A 52-year-old male ingested Sumithion and alcohol, presenting with somnolence but initially lacking typical organophosphate poisoning symptoms.
  • Fasciculations and salivation, characteristic early signs, appeared 40 hours post-ingestion.
  • Blood concentrations of Sumithion were monitored over time.

Findings:

  • Pharmacokinetic analysis using a 2-compartment model suggested retarded metabolism of Sumithion.
  • The delayed appearance of symptoms correlated with the observed slow metabolic clearance of the pesticide.

Related Experiment Videos

Implications:

  • Retarded metabolism of Sumithion can lead to a delayed onset of severe organophosphate intoxication.
  • This case underscores the need for prolonged monitoring in suspected organophosphate ingestions, even with an initial lack of symptoms.
  • Understanding individual metabolic variations is crucial for managing pesticide poisoning effectively.
