Background:

Obsessive compulsive disorders (OCD) encompass a spectrum of related conditions, including trichotillomania, tic disorders, and body dysmorphic disorder.
These disorders present with diverse manifestations, challenging traditional diagnostic boundaries.

Observation:

A case report details a 22-year-old Sri Lankan male with severe OCD presenting with life-threatening self-starvation.
The patient's atypical presentation, bordering on delusional psychosis, delayed diagnosis and treatment.

Findings:

Behavioral therapy, targeting OCD, proved successful in treating the patient's self-starvation.
Delayed diagnosis due to atypical presentation led to unnecessary investigations and a life-threatening condition.

Implications:

Psychological etiologies, particularly OCD, must be considered in cases of severe anorexia or self-starvation.
Early identification of feeding pattern disturbances linked to psychological causes can prevent diagnostic delays and critical health deterioration.
