Summary

Carbimazole, a medication for thyrotoxicosis, can induce systemic lupus erythematosus (SLE). This case highlights drug-induced SLE presenting as pleural effusion and a prothrombotic state.

Observation:

  • A 50-year-old female presented with dyspnea and pleuritic chest pain.
  • Investigations revealed sterile, exudative pleural effusion, elevated inflammatory markers, and new positive autoantibodies (ANA, anti-dsDNA, antihistone).
  • Echocardiography showed a left ventricular thrombus without myocardial infarction.

Findings:

  • The patient's thyrotoxicosis was being treated with carbimazole.
  • The clinical presentation and laboratory findings were consistent with drug-induced systemic lupus erythematosus (SLE).
