Background:

Budd-Chiari syndrome (BCS) is a rare condition characterized by hepatic venous outflow obstruction.
Postpartum BCS is exceptionally rare, posing diagnostic and therapeutic challenges.
Hypercoagulable states are known risk factors for venous thromboembolism.

Observation:

A patient presented with BCS two weeks after delivery.
The patient exhibited an unusual and prolonged state of hypercoagulability.
Despite aggressive anticoagulation therapy, extensive thrombosis developed.

Findings:

The case demonstrates severe hepatic vein occlusion.
Associated thromboses included renal vein thrombosis, inferior vena cava thrombosis, and femoral artery thrombosis.
