Summary

A persistent vulvar purpuric lesion in a Japanese woman was linked to chronic pelvic congestion. Histology showed hemorrhage and hemosiderin, suggesting increased venous pressure as a cause.

Area of Science:

  • Dermatology
  • Vascular Medicine
  • Pathology

Background:

  • Vulvar purpuric lesions can present asymptomatically, posing diagnostic challenges.
  • Understanding the etiology of chronic vulvar lesions is crucial for effective management.

Observation:

  • A 56-year-old Japanese woman presented with a long-standing, asymptomatic purpuric lesion on the vulva.
  • Clinical examination revealed a persistent, non-tender purpuric area.

Findings:

  • Histopathological examination demonstrated massive hemorrhage within the lesion.
  • Hemosiderin deposition and moderate lymphohistiocytic infiltration were noted.
  • The findings suggest a chronic vascular or hemorrhagic process.

Implications:

  • Chronic intrapelvic congestion, potentially due to abdominal ptosis, is hypothesized as a contributing factor.
  • Increased venous pressure may underlie the pathogenesis of persistent vulvar purpura.
  • This case highlights the importance of considering vascular etiologies in vulvar lesions.
