Background:

Bilateral habitual patellar dislocation is a rare condition.
Cleft lip and palate are common congenital anomalies.
Shortening of long bones (metatarsals, metacarpals, phalanges) can occur in various genetic syndromes.

Observation:

A unique case presenting with bilateral habitual patellar dislocation, cleft lip and palate, and significant limb shortening (brachydactyly) is described.
The patient exhibited recurrent, non-traumatic dislocation of both kneecaps.

Findings:

The association of habitual patellar dislocation with cleft lip and palate and limb shortening suggests a potential underlying genetic or developmental etiology.
Analysis of 20 previously observed cases provides a broader perspective on the possible causes and contributing factors.

Implications:

This case highlights the importance of considering a broader spectrum of skeletal and craniofacial anomalies in patients with patellar instability.
Understanding the etiological link may aid in early diagnosis and management of associated conditions.
Further research into the genetic and developmental pathways could elucidate the pathogenesis of this complex presentation.
