Chronic Bowel Disorders: Introduction

Chronic bowel diseases are a group of long-term conditions affecting the digestive tract, characterized by inflammation and damage to the gut lining. These conditions primarily include irritable bowel syndrome and inflammatory bowel disease.
Irritable Bowel Syndrome (IBS) is a common disorder affecting the gastrointestinal tract. The distinctive feature is recurrent abdominal pain associated with altered bowel movements, manifesting as constipation, diarrhea, or fluctuating between both. The...
Cholecystitis01:20

Cholecystitis

Cholecystitis is inflammation of the gallbladder, most commonly caused by obstruction of the cystic duct. This blockage prevents bile from draining, leading to gallbladder distension, inflammation, and potentially serious complications. This condition may present acutely or chronically and can happen with or without gallstones.EtiologyAbout 95% of cholecystitis cases are calculous, caused by gallstones blocking the cystic duct, leading to bile accumulation and inflammation of the gallbladder...

Area of Science:

  • Gastroenterology
  • Surgical Pathology

Context:

  • Biliary ileus is a rare complication of gallstones, occurring in 0.06% of patients.
  • It often presents as intestinal obstruction due to a cholecystoenteric fistula.
  • This condition is infrequently diagnosed, leading to high complication and mortality rates (12-27%).

Purpose:

  • To report the treatment experience of biliary ileus in a tertiary care hospital.
  • To analyze the clinical presentation, diagnosis, and management of biliary ileus.

Summary:

  • A 10-year retrospective study included 13 patients (mean age 57) with biliary ileus.
  • All patients presented with intestinal occlusion symptoms; preoperative diagnosis was achieved in 23%.
  • The primary surgical intervention was intestinal resection with anastomosis (54%), with an 8% mortality rate and a mean hospitalization of 15 days.

Impact:

  • Biliary ileus primarily affects older patients and should be suspected in cases of intestinal occlusion.
  • Early diagnosis facilitates timely intervention, potentially leading to less invasive surgical procedures.
  • Urgent laparotomy for intestinal occlusion is the mainstay of treatment, with biliary fistula management reserved for select cases.
