Cholecystostomy is a safe and effective procedure for critically ill elderly patients with gallbladder disease. This gallbladder decompression offers a curative solution when more extensive surgery poses prohibitive risks.

Area of Science:

  • Surgery
  • Gastroenterology
  • Critical Care Medicine

Background:

  • Cholecystostomy is often viewed as a palliative measure for acute cholecystitis in high-risk patients.
  • This study aimed to re-evaluate its efficacy in managing severe gallbladder disease.

Observation:

  • 37 elderly patients (average age 69) with severe gallbladder disease underwent cholecystostomy between 1974-1987.
  • 28 had acute destructive cholecystitis, often with complications like perforation or cholangitis; 27 were critically ill.
  • The procedure involved calculus extraction and drainage of the gallbladder and abscesses.

Findings:

  • The mortality rate was 5.4% (2 deaths).
  • Of 33 patients who had tube cholangiography, residual stones in 7 were managed electively later.

Related Experiment Videos

  • None of the 35 survivors experienced gallbladder disease symptoms during 1-12 years of follow-up.
  • Implications:

    • Cholecystostomy can be a curative operation for critically ill, elderly patients with gallbladder disease.
    • It provides a safe and effective alternative when extensive surgery is too risky.
