Malignant external otitis (MEO) can present with classic symptoms in healthy individuals. This condition often resolves favorably, even without antibiotic treatment, suggesting a milder disease course in some populations.

Background:

  • Malignant external otitis (MEO) is a severe infection typically affecting immunocompromised individuals.
  • Classic MEO symptoms include ear pain, discharge, and granulation tissue.

Observation:

  • Four healthy patients presented with suspected MEO and classic clinical signs.
  • Two patients received oral cautrimoxazole, while two did not receive any oral antibiotics.

Findings:

  • All four patients experienced a benign clinical course of the disease.
  • MEO demonstrated a favorable outcome in healthy individuals, irrespective of antibiotic intervention.

Implications:

  • Malignant external otitis may have a broader clinical spectrum in healthy individuals than previously recognized.
  • The findings suggest that antibiotics may not always be necessary for MEO in immunocompetent patients, warranting further investigation into conservative management strategies.
