Summary

Immunoglobulin G4-related disease (IgG4-RD) can present as peripheral neuropathy. Early diagnosis and treatment with oral prednisolone can improve symptoms of this IgG4-RD-associated neuropathy.

Background:

  • Immunoglobulin G4-related disease (IgG4-RD) is a newly identified condition characterized by elevated serum IgG4 levels and IgG4-positive plasma cell infiltration in tissues.
  • This case highlights IgG4-related disease presenting as peripheral neuropathy.

Observation:

  • A 55-year-old male patient presented with sensory-motor neuropathy, diagnosed as IgG4-related disease.
  • Neuropathic features included multiple mononeuropathies with axonal damage, and sural nerve biopsy revealed epineural thickening and IgG4-positive plasma cell infiltration.

Findings:

  • Histopathological examination of the sural nerve showed significant myelinated fiber loss without demyelination or vasculitis.
  • The patient experienced sensory-motor neuropathy with associated extremity pain and skin sclerosis.

Implications:

  • This case underscores the importance of considering IgG4-related disease in the differential diagnosis of neuropathy.
  • Prompt recognition and treatment with corticosteroids like oral prednisolone can lead to symptom amelioration in IgG4-RD-associated neuropathy.
