Summary

Pulmonary artery aneurysm (PAA) can cause bronchiectasis by compressing the left main bronchus. This rare complication, seen after a Senning procedure, necessitated a lobectomy despite medical treatment for pneumonia.

Background:

  • Pulmonary artery aneurysm (PAA) is a rare condition.
  • Transposition of the great vessels is a congenital heart defect often treated with the Senning procedure in infancy.
  • Bronchiectasis is an irreversible lung condition characterized by damaged airways.

Observation:

  • A case of bronchiectasis secondary to PAA compressing the left main bronchus is presented.
  • The PAA developed after a Senning procedure for transposition of the great vessels.
  • The patient experienced recurrent pneumonia due to the bronchial compression.

Findings:

  • This is believed to be the first reported case of bronchiectasis caused by a PAA.
  • Medical management for recurrent pneumonia was insufficient.
  • A lobectomy was ultimately required to address the bronchiectasis and prevent further complications.

Implications:

  • PAA should be considered in the differential diagnosis of bronchiectasis, especially in patients with a history of congenital heart surgery.
  • This case highlights a rare but serious complication of PAA.
  • Surgical intervention may be necessary for PAA-induced bronchiectasis when medical therapy fails.
