Postarthroscopy osteonecrosis of the knee

Background:

• Spontaneous subchondral osteonecrosis can occur in the knee after arthroscopic procedures.
• Meniscal tears, meniscectomy, and radiofrequency chondroplasty are potential causes of osteonecrosis.

Purpose of the Study:

• To determine if combining mechanical or radiofrequency chondroplasty with arthroscopic meniscectomy increases osteonecrosis incidence.
• To evaluate the etiological role of radiofrequency chondroplasty in knee osteonecrosis.

Main Methods:

• A prospective clinical trial involving 75 patients with knee cartilage degeneration (Stage II-III).
• Patients underwent arthroscopic partial meniscectomy; groups received additional mechanical debridement or radiofrequency (RF) chondroplasty.
• Preoperative and 6-month postoperative MRIs were used to assess for osteonecrosis development.

Main Results:

• Osteonecrosis developed in five patients post-surgery: two in the meniscectomy-only group, two in the meniscectomy with mechanical debridement group, and one in the meniscectomy with RF chondroplasty group.
• The addition of mechanical or RF chondroplasty did not elevate the incidence of osteonecrosis compared to meniscectomy alone.

Conclusions:

• Mechanical and RF chondroplasty, when combined with arthroscopic meniscectomy, do not increase the risk of developing osteonecrosis.
• Contrary to common assumptions, RF chondroplasty was associated with a reduced incidence of osteonecrosis in this study cohort.
