Low serum uric acid (SUA) levels are linked to better short-term recovery after acute ischemic stroke (AIS). However, SUA does not predict long-term stroke outcomes, suggesting it reflects stroke severity rather than prognosis.

Purpose of the Study:
- To investigate the prognostic significance of serum uric acid (SUA) levels in patients with acute ischemic stroke (AIS).
Main Methods:
- A cohort of 463 AIS patients from the PREMIER registry were analyzed.
- Multivariate models assessed the association between SUA and functional outcomes (modified Rankin Scale) at various follow-up points.
- A modified Rankin Scale score of 0-1 was defined as a very good outcome.
Main Results:
- Patients with lower SUA levels (≤4.5 mg/dl) showed a significantly higher rate of very good 30-day outcomes (30.5% vs. 18.9%).
- This association between low SUA and very good short-term outcome persisted after adjusting for multiple clinical factors (OR: 1.76, 95% CI: 1.05-2.95).
- SUA levels were not associated with mortality or functional dependence at 30 days or any outcome at longer follow-up intervals (3, 6, 12 months).
Conclusions:
- Low serum uric acid concentration is modestly associated with very good short-term outcomes following acute ischemic stroke.
- Findings suggest SUA may serve as a marker for the magnitude of cerebral infarction rather than an independent predictor of long-term stroke prognosis.
