  • Histologic assessment was performed on 25 biopsy specimens from the Achilles tendons.
  • Concordance between B-mode US and sonoelastography grading was analyzed using kappa (κ) statistics.
  • Main Results:

    • Sonoelastography accurately identified all 14 tendon thirds with histologic degeneration (100%).
    • B-mode US identified 12 of 14 (86%) degenerated tendon thirds.
    • A moderate agreement (κ = 0.52) was observed between B-mode US and sonoelastography.

    Conclusions:

    • Sonoelastography demonstrates higher sensitivity than B-mode US in detecting Achilles tendon degeneration.
    • This imaging technique may assist in predicting histopathologic signs of Achilles tendinosis.
    • Further research could validate sonoelastography's role in clinical diagnosis of tendinopathies.
