Periostin, a protein involved in fibrosis, is upregulated in degenerative intervertebral discs. This finding suggests periostin may serve as a prognostic marker and therapeutic target for disc degeneration.
Area of Science:
- Biochemistry
- Cell Biology
- Orthopedics
Background:
- Intervertebral disc degeneration is a complex condition often leading to chronic back pain.
- Periostin, an extracellular matrix protein, is implicated in fibrosis and mechanical stress responses.
- The role of periostin in disc degeneration has not been previously investigated.
Purpose of the Study:
- To compare gene expression profiles between degenerative and non-degenerative intervertebral discs.
- To understand the molecular mechanisms underlying disc degeneration.
Main Methods:
- Gene expression profiling using Affymetrix GeneChip Human arrays.
- Validation of periostin expression via RT-PCR, real-time RT-PCR, and Western blot.
- Immunohistochemistry and Gomori trichrome staining to assess periostin, fibrosis, and disc structure.
- Mechanical stress experiments on nucleus pulposus cells.
Main Results:
- Identified 14 genes with significantly different expression levels between degenerative and non-degenerative discs.
- Observed increased messenger RNA and protein levels of periostin in degenerative nucleus pulposus cells.
- Demonstrated increased periostin staining and fibrosis in degenerative discs, correlating with architectural disorder.
- Found that mechanical stress significantly induced periostin expression in degenerative cells but not in non-degenerative cells.
Conclusions:
- This study is the first to report periostin upregulation in human disc degeneration.
- Periostin, along with Type I collagen and matrix metalloproteinase-2, is upregulated in degenerative discs.
- Periostin shows potential as a novel prognostic marker and therapeutic target for disc degeneration.
