Fetal Circulation01:14

Fetal Circulation

Fetal circulation is a unique system that facilitates the exchange of gases, nutrients, and waste products between the developing fetus and the mother. This intricate process takes place through a special organ called the placenta.
Two umbilical arteries transport blood from the fetus to the placenta. At the placenta, the blood absorbs oxygen and nutrients while simultaneously eliminating waste products. This oxygen-enriched and nutrient-rich blood then returns to the fetus through one...

Area of Science:

  • Obstetrics and Gynecology
  • Neonatal Medicine
  • Clinical Chemistry

Background:

  • Fetal scalp sampling is used to assess fetal well-being during labor.
  • Umbilical cord blood analysis provides a direct measure of the neonate's acid-base status at birth.
  • Understanding the correlation between fetal scalp and cord blood parameters is crucial for accurate neonatal outcome prediction.

Purpose of the Study:

  • To evaluate the correlation between fetal scalp base excess (BE) and umbilical cord BE.
  • To assess the predictive value of fetal scalp pH, BE, and lactate for neonatal metabolic acidosis.

Main Methods:

  • A retrospective study analyzed fetal scalp and umbilical cord samples from 71 deliveries.
  • Fetal heart rate abnormalities (FHRA) were classified.
  • Correlations between fetal scalp and cord samples for pH, BE, and lactate were calculated.
  • Receiver operating characteristic (ROC) curves were used to evaluate predictive values.

Main Results:

  • Significant correlations were found between fetal scalp and cord samples for pH (r=0.23), BE (r=0.49), and lactate (r=0.52).
  • The time interval between sampling and delivery, and the severity of FHRA did not significantly impact the results.
  • ROC curves indicated a similar, poor predictive pattern for all tested parameters.

Conclusions:

  • Fetal scalp and umbilical cord samples for pH, BE, and lactate show significant correlation.
  • The predictive value of fetal scalp parameters for neonatal metabolic acidosis (cord pH ≤ 7.20) is limited.
