Managing malignant insulinoma, a rare pancreatic tumor, is challenging. Multidisciplinary approaches involving various therapies are essential for controlling persistent hypoglycemia in these patients.
Area of Science:
- Endocrinology
- Oncology
- Surgical Oncology
Background:
- Malignant insulinoma is a rare, functional endocrine pancreatic tumor.
- Effective management poses significant challenges for oncologists and endocrinologists.
Observation:
- A retrospective review of seven malignant insulinoma patients (1995-2011) was conducted.
- Six tumors were sporadic, and one was associated with multiple endocrine neoplasia type 1 (MEN-1).
- Surgery was performed in six patients; one was unresectable.
Findings:
- All patients experienced persistent hypoglycemia, requiring diverse treatments including somatostatin analogs, glucocorticoids, diazoxide, and everolimus.
- Other interventions included chemoembolization and yttrium-90 radioembolization.
- A successful liver transplant was performed in the MEN-1 patient.
Implications:
- Hypoglycemia management in malignant insulinoma is complex and necessitates a multimodal therapeutic strategy.
- Further research is required to establish optimal treatment protocols for this rare condition.
