Background:

The nursing intellectual capital theory posits that nursing knowledge influences patient and organizational outcomes.
It suggests nursing human capital (knowledge, skills, experience) affects patient care quality, nurse recruitment, and retention.
Work environment factors like nurse staffing and employer support for continuing professional development are proposed to mediate these effects.

Purpose of the Study:

To empirically test key propositions of the middle-range theory of nursing intellectual capital.
To examine the relationships between nursing human capital, work environment factors, and patient/organizational outcomes.

Main Methods:

A cross-sectional survey design was employed in six Canadian acute care hospitals in 2008.
Data were collected from 91 inpatient care units using clearly specified empirical indicators for study variables.
Structural equation modeling was used to test the theory's propositions with the collected data.

Main Results:

Propositions related to nursing human capital were supported.
Propositions concerning employer support for nurse continuing professional development were not supported.
The mediating influence of nursing human capital on the relationship between nurse staffing and patient outcomes was partially supported.

Conclusions:

The study provides partial empirical validation for the nursing intellectual capital theory.
Further theoretical refinement is needed for operationalizing and measuring certain concepts.
Additional research in diverse settings is recommended to further scrutinize the theory.
