Core binding factor acute myeloid leukaemia (CBF AML) shows a favorable prognosis, but c-KIT mutations did not impact survival in this study. Higher lactate dehydrogenase (LDH) levels were observed in patients with c-KIT mutations, suggesting increased proliferation.
Area of Science:
- Hematology
- Oncology
- Molecular Biology
Background:
- Core binding factor acute myeloid leukaemia (CBF AML) is a subtype with a generally good prognosis.
- Activating c-KIT mutations are implicated in increased relapse risk and reduced survival in some AML subtypes.
- The prognostic significance of c-KIT mutations in CBF AML requires further investigation.
Purpose of the Study:
- To determine the incidence of c-KIT mutations in de novo CBF AML.
- To evaluate the prognostic value of c-KIT mutations on overall survival (OS) and disease-free survival (DFS) in CBF AML.
- To explore associations between c-KIT mutations and clinical parameters.
Main Methods:
- Retrospective analysis of 23 adult de novo CBF AML patients (14 inv(16), 9 t(8;21)).
- Analysis of c-KIT mutations in specific exons (8, 9, 10, 11, 13, 14, 17) using PCR and direct sequencing.
- Correlation of mutation status with clinical data, including cytogenetics, remission, WT1 levels, LDH, OS, and DFS.
Main Results:
- c-KIT mutations were detected in 7/23 (30.4%) patients (4 with inv(16), 3 with t(8;21)).
- No significant difference in c-KIT mutation status was found based on specific CBF AML subtypes or additional cytogenetic abnormalities.
- Higher lactate dehydrogenase (LDH) levels were observed in c-KIT mutated patients (p=0.01), but OS and DFS were similar between mutated and non-mutated groups.
Conclusions:
- CBF AML maintains a better prognosis than other AML types, and inv(16) AML shows better DFS than t(8;21) AML.
- c-KIT mutational status did not demonstrate prognostic value for OS or DFS in this cohort of CBF AML patients.
- Elevated LDH in mutated CBF AML suggests potentially more active cellular proliferation.
