Background:

  • Computer-aided detection (CAD) systems are used to assist radiologists in identifying abnormalities.
  • Lung nodule detection on multidetector computed tomography (CT) is a critical diagnostic task.
  • Evaluating different CAD modes, such as concurrent-reader and second-reader, is essential for optimizing workflow and performance.

Purpose of the Study:

  • To compare the reading times and detection performances of radiologists using concurrent-reader and second-reader CAD modes.
  • To assess the efficiency and accuracy of different CAD system configurations for lung nodule detection.

Main Methods:

  • Retrospective analysis of 50 multidetector CT datasets with 207 confirmed lung nodules.
  • Six radiologists (3 experienced, 3 residents) independently interpreted datasets using both concurrent-reader and second-reader CAD modes.
  • Jackknife free-response receiver operating characteristic (JAFROC) analysis was employed to compare detection performances.

Main Results:

  • Mean reading time was significantly shorter with concurrent-reader CAD (132s) compared to second-reader CAD (210s) (p<0.01).
  • No significant difference in detection performance was observed between the two CAD modes (JAFROC figure-of-merit: 0.70 for concurrent-reader vs. 0.72 for second-reader, p=0.35).

Conclusions:

  • Concurrent-reader CAD mode offers a significant time-saving advantage for radiologists interpreting lung nodules on multidetector CT.
  • The choice between concurrent-reader and second-reader CAD modes does not appear to impact the overall detection performance of radiologists.
