Background:

The calcifying odontogenic cyst (COC), first described in 1962, is a rare developmental odontogenic cyst.
It is characterized by a cystic lining with "Ghost" epithelial cells and calcification potential.
COCs can present with diverse clinical, histopathological, and radiological features, sometimes associated with other odontogenic tumors.

Observation:

This article details a case of an asymptomatic calcifying odontogenic cyst.
The initial presentation was misleading, delaying the correct diagnosis.
Histopathological examination revealed the COC alongside features suggestive of an adenomatoid odontogenic tumor.

Findings:

The study presents a rare instance of COC with co-existing features resembling an adenomatoid odontogenic tumor.
The asymptomatic nature of the lesion complicated the diagnostic process.
Histopathology was crucial in identifying the dual nature of the odontogenic lesion.

Implications:

This case underscores the importance of comprehensive histopathological analysis for accurate diagnosis of odontogenic cysts and tumors.
Recognizing the spectrum of COC variants and their potential association with other tumors is critical for effective management.
Further research into the transformation pathways and co-occurrence of COC with other odontogenic lesions is warranted.
