Background:

Acquired partial lipodystrophy (PLD) is a rare disorder characterized by selective loss of subcutaneous fat.
Retinal complications, including drusenoid lesions, have been reported in PLD patients.

Observation:

A 55-year-old woman with known PLD presented with sudden vision loss in her right eye.
Ocular examination revealed intraretinal edema indicative of choroidal neovascularization (CNV).

Findings:

Diagnostic imaging confirmed CNV as the cause of visual impairment.
Intravitreal ranibizumab treatment led to excellent visual recovery and sustained vision for 4 years.

Implications:

This is the first reported case of CNV associated with PLD.
Highlights the importance of considering CNV in the differential diagnosis of acute vision loss in patients with PLD and drusenoid lesions.
