Summary
Prior laparoscopic surgery experience does not significantly impact laparoscopic suturing skill acquisition. Both experienced and novice surgeons improve with training, with novices catching up to experienced surgeons by the end of the study.
Area of Science:
- Surgical Education
- Minimally Invasive Surgery
Background:
- Laparoscopic suturing is a complex surgical skill requiring extensive training.
- Mastery of laparoscopic techniques is crucial for effective minimally invasive surgery.
Purpose of the Study:
- To evaluate if prior laparoscopic surgery exposure accelerates the learning curve for laparoscopic suturing.
- To compare skill acquisition rates between laparoscopy-exposed and laparoscopy-naïve surgical trainees.
Main Methods:
- Twenty laparoscopy-exposed and 20 laparoscopy-naïve surgeons performed 5 laparoscopic gastrojejunostomies each on a porcine model.
- Performance was assessed by operation time, anastomotic score (leak, size, suture placement, mucosal approximation), and perceived difficulty.
- Baseline performance was compared to post-training performance.
Main Results:
- All surgeons demonstrated significant improvement in operation time, anastomotic score, and reduced difficulty.
- Initially, laparoscopy-exposed surgeons had faster operation times, but this difference diminished with training.
- No significant difference in overall anastomotic score was observed between groups by the end of training.
Conclusions:
- Surgical training effectively enhances laparoscopic suturing skills for both novice and experienced surgeons.
- Prior laparoscopic experience does not confer a lasting advantage in acquiring suturing skills, as novices achieve comparable proficiency.
- Laparoscopy-naïve surgeons successfully bridge the skill gap with dedicated training.
