Background:

Current primary care for older, vulnerable patients is reactive and fragmented.
There is a growing need for proactive and integrated care models for an increasing frail elderly population living at home.

Purpose of the Study:

To explore two innovative interventions in primary care for frail elderly patients.
To evaluate the effectiveness of a new screening and monitoring program and a multidisciplinary intervention program.

Main Methods:

A three-armed, cluster randomized trial involving 58 primary care practices.
Interventions included a screening/monitoring program and a multidisciplinary nurse-led program for frail elderly patients.
The study included 3,008 patients, with data collected from Utrecht, the Bilt, and Maarsenbroek.

Main Results:

The primary outcome measured was the impact of interventions on daily activities using the Katz questionnaire.
Secondary outcomes included quality of life, mortality, and healthcare utilization (care/nursing home, emergency room, hospital admissions).
Data on volunteer caregiver support was also collected.

Conclusions:

The study provides insights into the effectiveness of proactive and integrated care models for frail elderly individuals.
Findings will inform the development of improved primary care strategies for the aging population.
