Repeat hepatectomy for colorectal metastases offers long-term survival benefits in select patients. This procedure shows comparable morbidity to initial liver resection, with improved survival rates for those undergoing repeat operations.

Background:

The oncological benefit of repeat hepatectomy for recurrent colorectal metastases remains uncertain.
This study evaluates the efficacy of repeat hepatectomy within contemporary multidisciplinary treatment protocols.

Purpose of the Study:

To assess the oncological value and outcomes of repeat hepatectomy for patients with recurrent colorectal metastases.
To compare the postoperative outcomes and survival rates of patients undergoing repeat hepatectomy versus a single hepatectomy.

Main Methods:

A retrospective analysis of consecutive patients who underwent repeat hepatectomy for colorectal metastases between 1990 and 2010.
Exclusion of patients who underwent two-stage hepatectomy.
Comparison of postoperative outcomes and survival data between patients with single and repeat hepatectomies.

Main Results:

Repeat hepatectomy (second, third, or fourth) had similar postoperative morbidity compared to initial liver resection.
Postoperative mortality was slightly higher after repeat hepatectomy (3.1%) versus first hepatectomy (1.6%).
Three- and 5-year survival rates were significantly higher in patients who underwent repeat hepatectomy (76% and 54%) compared to those with a single hepatectomy (58% and 45%). Repeat hepatectomy between 2000-2010 was an independent predictor of longer survival.

Conclusions:

Repeat hepatectomy is a viable option for selected patients with recurrent colorectal metastases.
This procedure can offer significant long-term survival advantages in carefully chosen patient cohorts.
