Summary

Isolated Achilles tendinitis can be the sole long-term symptom of Human Leukocyte Antigen B27 (HLA-B27) associated disease. This finding suggests a distinct disease entity may exist, presenting solely with tendinitis.

Background:

Human Leukocyte Antigen B27 (HLA-B27) is strongly associated with seronegative spondyloarthropathies.
Achilles tendinitis is a common manifestation in these conditions.

Observation:

A case report details a patient with a 15-year history of isolated bilateral Achilles tendinitis.
The patient exhibited no other clinical features of seronegative spondyloarthropathies.

Findings:

This presentation suggests that Achilles tendinitis can be the sole, long-standing clinical manifestation of an HLA-B27-associated disease.
The findings challenge the typical presentation patterns of HLA-B27 related conditions.

Implications:

Related Experiment Videos

The possibility of a distinct HLA-B27-associated disease entity presenting primarily with tendinitis warrants further investigation.

This case expands the clinical spectrum recognized for HLA-B27 associations.

Consideration of HLA-B27 testing in cases of isolated, persistent Achilles tendinitis may be beneficial.
