The oncoplastic approach in breast conservation therapy (BCT) reduces positive margins and improves patient satisfaction. While local recurrence rates need further study, this method offers cosmetic and oncologic benefits.
Area of Science:
- Oncology
- Plastic Surgery
- Breast Surgery
Background:
- Breast conservation therapy (BCT) is a standard treatment for breast cancer.
- Immediate reconstruction in BCT can improve cosmetic outcomes beyond initial expectations.
Purpose of the Study:
- To compare outcomes between BCT alone and BCT with an oncoplastic approach.
- To evaluate the oncologic and aesthetic results of oncoplastic breast surgery.
Main Methods:
- A meta-analysis was conducted using PubMed, searching for "oncoplastic," "partial breast reconstruction," and "breast conservation therapy."
- Studies with less than 10 patients or under 1-year follow-up were excluded.
- Three groups were compared: BCT with oncoplastic reduction, BCT with oncoplastic flaps, and BCT alone.
Main Results:
- The oncoplastic group (3165 patients) showed a significantly lower positive margin rate (12% vs. 21%) compared to BCT alone (5494 patients).
- Re-excision was more common in the BCT alone group (14.6% vs. 4%), but completion mastectomy was more frequent in the oncoplastic group (6.5% vs. 3.79%).
- Patient satisfaction with aesthetic outcomes was significantly higher in the oncoplastic group (89.5% vs. 82.9%).
Conclusions:
- The oncoplastic approach facilitates wider tumor resection, leading to reduced positive margins in BCT.
- While local recurrence requires further investigation, oncoplastic surgery significantly enhances patient satisfaction with cosmetic results.
