Cardiomyopathy V: Interprofessional Care

Managing cardiomyopathy involves addressing underlying or precipitating causes, treating heart failure with medications, and implementing dietary changes and a balanced exercise and rest regimen.Lifestyle ModificationsCardiomyopathy patients should adopt a low-sodium diet to reduce fluid retention and manage heart failure. A personalized exercise and rest plan helps maintain physical fitness without overstraining the heart. Avoiding alcohol and tobacco is essential to prevent further damage to...

Cardiac Catheterization II: Right Heart Catheterization

Right Heart Catheterization: An OverviewRight heart catheterization is an invasive diagnostic procedure that measures right-sided cardiac and pulmonary artery pressures, calculates cardiac output, and identifies intracardiac shunts. It provides detailed hemodynamic data essential for diagnosing and managing various cardiovascular conditions, such as pulmonary hypertension.Access SitesCommon access sites for right heart catheterization include the internal jugular vein in the neck region, the...
Continuous Renal Replacement Therapy01:30

Continuous Renal Replacement Therapy

Continuous Renal Replacement Therapy, also known as CRRT, is a procedural treatment for acute kidney injury (AKI) that gradually removes uremic toxins and fluids while maintaining acid-base balance and stabilizing electrolytes. It is particularly useful for hemodynamically unstable patients. Unlike intermittent hemodialysis, which is faster, CRRT provides a gentler approach over 24 hours, closely mimicking the function of natural kidneys. However, CRRT is not ideal for patients with...

Heart Failure VI: Adjunct Therapies

Additional therapies for treating patients with heart failure (HF) may include procedural interventions, supplemental oxygen, the management of sleep disorders, and nutritional therapy.Procedural InterventionsImplantable Cardioverter-Defibrillator: For patients at risk of life-threatening arrhythmias due to severe left ventricular dysfunction, an Implantable Cardioverter-Defibrillator (ICD) can detect and terminate these arrhythmias, preventing sudden cardiac death and improving survival rates.

Summary

Acute right ventricular failure is rare. A HeartWare HVAD was successfully used for isolated right heart support in a patient with myocardial infarction, demonstrating a novel treatment option.

Area of Science:

  • Cardiology
  • Cardiovascular Surgery
  • Biomedical Engineering

Background:

  • Acute isolated right ventricular (RV) failure secondary to myocardial infarction is uncommon.
  • Currently, no dedicated assist devices are approved for permanent isolated RV support.
  • Management of RV infarction often relies on supportive measures.

Observation:

  • A patient experienced extensive RV myocardial infarction due to iatrogenic right coronary artery dissection.
  • The HeartWare HVAD system was implanted for isolated RV mechanical support.

Findings:

  • The implantation was successful, providing effective isolated RV assistance.
  • The device seamlessly integrated with the pulmonary circulation without requiring system modifications.

Implications:

  • This case demonstrates the potential utility of existing ventricular assist devices for off-label isolated RV support.
  • Successful HVAD implantation offers a potential therapeutic option for rare cases of RV failure unresponsive to conventional treatments.
