  • Synthesized 96 radiographs using a THR simulator with varying femoral inclination and version.
  • Measured synthetic and 28 real radiographs using Elliversion and trigonometric methods, assessing repeated measurements and inter-measurement differences.
  • Main Results:

    • Elliversion software demonstrated significantly higher precision than the trigonometric method for synthetic radiographs with the femoral head included (p < 0.01).
    • No significant difference was found between methods for synthetic radiographs without the femoral head (p = 0.19).
    • For real radiographs, Elliversion showed significantly better precision when excluding poor-quality images (p = 0.04).

    Conclusions:

    • The Elliversion software, a computerized ellipse method, provides superior precision for measuring acetabular version compared to the trigonometric method.
    • This enhanced precision is particularly evident with good-quality radiographs.
    • Improved acetabular version measurement accuracy can facilitate earlier diagnosis of acetabular loosening.
