Summary

Dacrystic seizures, characterized by crying, are rare epilepsy symptoms. This study details the first instance of inducing such seizures via direct brain stimulation in a patient with left temporal lobe epilepsy.

Observation:

  • A 41-year-old female patient with medically intractable left temporal lobe epilepsy underwent intracranial video-EEG monitoring.
  • Cortical stimulation was applied to the left posterior orbito-frontal gyrus during extraoperative monitoring.

Findings:

  • Direct cortical stimulation of the left posterior orbito-frontal gyrus reproducibly elicited dacrystic seizures with clear affective content.
  • This represents the first reported case of inducing dacrystic seizures through direct brain stimulation.

Implications:

  • This finding suggests the left posterior orbito-frontal gyrus plays a crucial role in the generation of dacrystic seizures.
  • It may offer new insights into the neural circuitry of emotional expression and epilepsy.
  • Potential implications for surgical targeting and understanding non-lesional epilepsy.
