    Background:

    • Bile reflux and aspiration can lead to significant respiratory complications.
    • Previous studies indicate intratracheal bile instillation causes pulmonary damage in animal models.

    Purpose of the Study:

    • To evaluate the dose-dependent pulmonary toxicity of aspirated bile in a rabbit model.
    • To assess the effects of varying bile concentrations and hydrochloric acid on lung tissue.

    Main Methods:

    • Rabbits underwent intratracheal instillation of different concentrations of bile (physiological to 100%), 1% bile salt solution, and hydrochloric acid (pH 1.0).
    • Survival rates and pulmonary pathology, including edema, atelectasis, and hemorrhage, were assessed.

    Main Results:

    • Intratracheal bile instillation induced severe pulmonary edema, atelectasis, and focal hemorrhages.
    • Concentrations of bile exceeding 3% resulted in 100% mortality in the test animals.
    • Hydrochloric acid and bile salt solutions also caused significant pulmonary damage.

    Conclusions:

    • Aspiration of bile, particularly at higher concentrations, is associated with severe and potentially fatal pulmonary injury.
    • While direct extrapolation to humans requires caution, the experimental findings strongly suggest bile is a dangerous aspirate.
    • These results underscore the importance of preventing bile aspiration in clinical settings.
