Oral direct factor Xa inhibitors like apixaban and rivaroxaban significantly outperform enoxaparin in preventing deep vein thrombosis (DVT) after hip or knee replacement surgery. Safety outcomes remain comparable across treatments.

Background:

  • Venous thromboembolism (VTE) is a significant risk following major orthopedic procedures such as total hip arthroplasty (THA) and total knee arthroplasty (TKA).
  • Direct oral anticoagulants, specifically factor Xa (FXa) inhibitors, have emerged as alternatives to traditional therapies like enoxaparin for VTE prophylaxis.

Purpose of the Study:

  • To compare the efficacy and safety of oral FXa inhibitors (apixaban, rivaroxaban) versus enoxaparin for VTE prevention after THA and TKA.
  • To analyze key outcomes including deep vein thrombosis (DVT), pulmonary embolism (PE), and bleeding events.
