Does left ventricular aneurysm influence survival after acute myocardial infarction?

- A cohort of 386 male patients under 60 years old, post-AMI, were followed for a median of 57 months.
- Left ventricular aneurysm was diagnosed via catheterization in 52 patients (13.5%).
- Cox regression analysis was used to identify independent predictors of survival, with matched-pair analysis performed for LVA patients.
Main Results:
- Initially, patients with LVA showed higher mortality (29% vs 14%).
- However, after adjusting for ejection fraction, bifascicular block, diseased vessels, and post-infarction angina, LVA was not an independent predictor of survival.
- Matched-pair analysis confirmed similar survival rates between LVA patients and controls (75% vs 82% mortality).
Conclusions:
- Left ventricular aneurysm is not an independent predictor of mortality after acute myocardial infarction.
- Ejection fraction, bifascicular block, number of diseased vessels, and angina are significant independent predictors of survival post-AMI.
