Summary

Performance on the Paced Auditory Serial Addition Test (PASAT) in multiple sclerosis (MS) patients correlates with cerebral blood flow (CBF) in the left centrum semiovale. This suggests a link between blood flow and cognitive function in MS.

Background:

  • Multiple Sclerosis (MS) is a demyelinating disease affecting white matter.
  • Cognitive impairment, particularly in processing speed, is common in MS.
  • The Paced Auditory Serial Addition Test (PASAT) is a common cognitive assessment in MS.

Purpose of the Study:

  • To investigate the relationship between PASAT performance and cerebral blood flow (CBF).
  • To assess the association between PASAT performance and axonal metabolic integrity in normal-appearing white matter (NAWM).
  • To explore these relationships in the centrum semiovale of MS patients.

Main Methods:

  • Magnetic resonance (MR) imaging was used in 28 individuals (18 MS patients, 10 controls).
  • Cerebral blood flow (CBF) was measured using pseudo-continuous arterial spin labeling.
  • Axonal metabolic integrity (NAA/Cr ratios) was assessed via (1)H-MR spectroscopy in normal-appearing white matter (NAWM) of the centrum semiovale.

Main Results:

  • A positive correlation was found between PASAT performance and CBF in the left centrum semiovale in MS patients (P = 0.008).
  • No significant correlations were observed between PASAT scores and NAA/Cr ratios in MS patients.
  • PASAT scores did not correlate with CBF in the right centrum semiovale, frontoparietal cortices, or cerebellar hemispheres.

Conclusions:

  • PASAT performance in MS patients is associated with CBF in the left centrum semiovale.
  • This finding highlights the role of cerebral blood flow in cognitive function, specifically information processing speed and working memory, within MS-related white matter tracts.
