Urologic Endoscopic Procedure: Cystoscopic Examination01:28

Urologic Endoscopic Procedure: Cystoscopic Examination

Meaning of Cystoscopic Examination:Cystoscopy is an essential diagnostic tool in urology that is used to assess the structure and function of the genitourinary system. It provides a direct view of the urethra, bladder, and, in some cases, the ureteral openings. This procedure helps detect structural abnormalities, infections, cancers, and blockages in the urinary tract. There are two types of cystoscopy:Flexible cystoscopy is commonly performed in outpatient settings due to its less invasive...

Imaging Studies VI: Voiding Cystourethrography and Cystography

Voiding Cystourethrography (VCUG) and Cystography are specialized radiographic procedures used to examine the structure and function of the bladder and urethra.Voiding Cystourethrography (VCUG)A Voiding Cystourethrogram (VCUG) is a diagnostic imaging procedure that assesses the anatomy and function of the lower urinary tract. It focuses on the bladder, bladder neck, and urethra, helping detect abnormalities such as vesicoureteral reflux (VUR)—the backward or reverse flow of urine into the...

Area of Science:

  • Urology
  • Surgical Education
  • Medical Simulation

Background:

  • Virtual reality (VR) simulators offer a novel approach to surgical skills training.
  • They enable simulation of diverse surgical scenarios and objective evaluation of trainee performance.
  • Existing simulators for transurethral resection of the prostate (TURP) are available, prompting investigation into their utility.

Purpose of the Study:

  • To investigate the effectiveness of a virtual reality simulator for training transurethral prostatectomy (TURP) skills.
  • To assess the impact of simulation-based training on surgical performance and safety parameters.

Main Methods:

  • Thirty-eight urologists participated in TURP training using the TURPSim™ system.
  • Pre- and post-training assessments included the global rate scale (GRS) and simulator-recorded objective parameters.
  • Participant feedback was collected via questionnaires.

Main Results:

  • Post-training, GRS scores significantly improved (18.0 ± 4.0 vs. 12.4 ± 4.2, P < 0.001).
  • Key surgical parameters improved, including reduced capsule resection rate (26.3% ± 0.6% vs. 21.2% ± 0.4%, P < 0.001), decreased blood loss ((125.8 ± 86.3) ml vs. (83.7 ± 41.6) ml, P < 0.001), and less external sphincter injury (3.6 ± 2.9 vs. 2.0 ± 2.0, P < 0.001).
  • The majority of trainees reported satisfaction, finding the simulator realistic and beneficial for skill enhancement.

Conclusions:

  • TURP training using virtual simulators enhances urologists' surgical skills and improves patient safety.
  • The TURPSim™ system shows promise for application in urologic surgery education and training.
  • Simulation-based training is a valuable adjunct for developing proficiency in transurethral prostatectomy.
